New Patient Information
For your first visit, you’ll need to complete several forms, including one for patient information and medical history, plus forms for our privacy and financial policies. Dr. Ching will then conduct a thorough clinical and radiographic exam to help you diagnose your needs. Based on your diagnosis, we’ll present you with the most appropriate treatment options for your situation. If you elect to proceed with endodontic therapy, we’ll schedule an appointment for you and talk to you about the important financial information you need to know.
Financial Policy
Our fees are determined by the complexity of the procedure and the tooth (or teeth) we’ll be working on. Our staff will provide you with an accurate estimate after your initial evaluation. Most dental insurance policies provide some coverage for endodontic care. We also accept payment through Care Credit, which offers extended payment plans with low fixed monthly payments. Care Credit is designed to help patients pay for care not covered by insurance and offers both no interest and low-interest payment plans to qualifying patients.
